Standardise Scores

I am attempting to workout how certain standardise scores have be calculated.

The values and scores that I have been provided are below. When I attempt to get the same scores using excel I get different figures.

The information I have been given stated that they use a scale of 0 to 4 for the scores so I was simply adding 2 to my calcuations - Not sure if this would be the correct way to adjust the scale from the normal -2 to 2 scale.

Any help of how I can calculate the below scores from these values would be great!
