Conversion of y=2X^2 + 4 to polar coordinates

\(\displaystyle x= r cos(\theta)\), \(\displaystyle y= r sin(\theta)\)
so \(\displaystyle y= 2x^2+ 4\) becomes \(\displaystyle r sin(\theta)= 2r^2 cos^2(\theta)+ 4\).
 
Can Some one help me converting
y = 2x2+4
in polar form???:confused:
