Need help with row operations

I've been trying to do these all day and I keep getting the wrong answers. Can someone help me out? I attached the problem I'm doing, and my attempt at solving it. I'm not sure how readable my work is but I keep getting the same result, not sure how to proceed.

From the 3rd matrix to the 4th matrix you noted that you were just changing row 1 but clearly you did more. Can you please note what changes you are about to do to one matrix to get the next matrix. Also row 1 in the 4th matrix is wrong.
 
I figured out my mistake. On the third matrix in that picture I added R2 + 2R1, and the last value in the column was a 0 added to 2*2 (=4), hence I changed the value without adding anything. That's a mistake, right? Pretty sure because I did the problem again and solved it.
