I think it is asking "what one number when multiplied by each of the numbers in the first set, produces the corresponding number in the second set?"
It may be easier to start with the last number and work backward.

Yes, Input/Output is a peculiar format for "writing a rule for a fraction". It would make more sense to me if input and output were reversed.

Then the rule would be "multiply the input by 1/5".
