The general equation of a quadratic is:
y = A*x2 + B*x + C
You are given co-ordinates of 3 points on the graph. From that you can write 3 equations (using the equation above) and solve for three unknowns (A, B & C).

Here's another way.
You know the two x-intercepts and the y-intercept. You can write an equation in factored form using the x-intercepts, and choose the coefficient to make the y-intercept right.
